The Mission of the Hope Clinic and Care Center is to partner with the community to help improve it’s health and well-being by providing access to quality health care for the healing of the mind, body and spirit by demonstrating the love of Jesus Christ to those in need.

Serving the Need

Millions of people in the U.S., many of whom are our neighbors, friends, and family, lack the blessing to access to primary healthcare. This includes those who are uninsured, underinsured, facing language barriers, geographical barriers, cultural barriers, and lack of availability of quality healthcare services. Founded in 2015 and located within the Goodwill Headquarters for NE Wisconsin in Menasha, Hope Clinic and Care Center (HCC) partners with the surrounding communities to provide collaborative wraparound support to over 700 patients annually, each of whom are living at or below 300% of the Federal Poverty Level. The clinic offers primary care, specialty care, labs, medications, counseling, and social assistance referrals and navigation to rent support, utilities, dental, vision, job placement and training, and more.

By addressing the immediate healthcare needs of patients, Hope Clinic helps to break the cycle of poor health and limited access to care and improves the health and quality of life for those who might otherwise go without care. Through meticulous effort, we hope to make a positive impact on the health and well-being of the communities it serves and works to create a more equitable and just future.

Services

HEALTHCARE
Primary care, medication support, labs, and specialty care including: dermatology, orthopedics, physical therapy, and more. Our Care Management focuses on goals set by primary care providers and patients to help improve their health, including reducing blood pressure concerns through the self-measured blood pressure program.
COUNSELING
Counseling services, a growing need in our area, are available to patients of all ages and provided by masters-trained staff.
CARE NAVIGATION WITH SOCIAL ASSISTANCE
Care Navigation team members work to overcome patient barriers to health, including: housing, food, transportation, jobs, dental, and vision.

Appleton Alliance Church
